Advice on best rhinoplasty surgeon please!!!!!!!!!!

I have wanted rhinoplasty for a very very long time and finally have the funds and am ready to do it! I just need help choosing a doc. I have narrowed it down to a list of 11( actually 12, I am adding Ivo Pitanguy... ridiculous I know but why not!)
I am leaning to Dr. Francis Palmer, I am just concerned that I cant find any reviews for him good or bad, nor any pictures...is that a good sign?

On the opposite side of the spectrum, with someone like Toriumi, there are so many negative comments online against him, I was originally leaning towards him but now I'm just confused about his abilities as a primary rhinoplasty surgeon...any thoughts? So many rate him the best but then there are many victims online telling the world he is the worst...what is the truth?

Here is what I am looking for in a surgeon - top expertise/ability in Rhinoplasty, values natural looking noses that wont collapse in 10 years, a perfectionist (I'm sure that comment is going to get me in trouble, but it is true, I would like to find a perfectionist), someone who wont go crazy with the knife and do anything we havent discussed, nice attitude, someone who will listen to all my worries and hopes without rushing me out for their next celebrity client, and someone who loves their job and views it as a craft and understands the utmost important role they are playing in our lives as a surgeon on our face for pete's sake!!!...not to much to ask for, I hope!:):):):)

Any help, feedback, especially real experiences with pics before and after is greatly appreciated. I would get my surgery tomorrow if I could narrow down the search to the best doc for me but I need your help.

Dr. Ivo Pitanguy - Brazil
Dr. Dean Toriumi - IL
Dr. Frank Ryan - LA
Dr. Francis Palmer - LA -
Dr. Portuese - Seattle -
Dr. Norman Pastorek - NY-
Dr. Jay Calvert - LA -
Dr. Andrew Frankel - LA -
under lesser consideration but still definitely on my list
Dr. Greg Mueller - LA -
Dr. Mark Epstein -NY- LI
Dr. Simoni - LA
Dr. Calvin Johnson - LA

Thank you!!!!!!!!!

Hi Anna,

I like how you say you are looking for "the best", but that qualifier is subjective. Several factors come into play including if is it a primary procedure or revision (primary for you), if the surgeon specializes in a particular style of rhinoplasty (i.e.: Asian, ethnic, etc.), comfortability with surgeon (can you compromise utilizing your personal research and the surgeon's expertise), and meeting real live patients (not actors or friends of the surgeon).

If you can get your surgery in the US, you should. Docs out of the country do NOT have to abide by the same jurisdictions, education levels, and medical regulations. This is for your protection. As an individual who was born outside the US, I can tell you all qualifications vary from place to place.

Your confusion about Toriumi is understandable. I have seen some very good work done by him. I have also heard some horror stories (not seen). Keep in mind he loves to stay in the public eye for self-promotion purposes. He is a mediawhore.

You cannot go by mere opinions on the internet. Even people on this site providing before and afters might have their own agendas. You are asking a lot of questions (which is a very good start) but you need to use internet sites, forums, and pics as a guide. Not as the conclusive end all, be all.

Palmer does have a website. He also has an interview posted on this forum. He does a lot of procedures and performs telephone consults. I have seen work done by him on several different individuals. Personally, I was never impressed.

You said: "Here is what I am looking for in a surgeon - top expertise/ability in Rhinoplasty, values natural looking noses that wont collapse in 10 years, a perfectionist (I'm sure that comment is going to get me in trouble, but it is true, I would like to find a perfectionist), someone who wont go crazy with the knife and do anything we havent discussed, nice attitude, someone who will listen to all my worries and hopes without rushing me out for their next celebrity client, and someone who loves their job and views it as a craft and understands the utmost important role they are playing in our lives as a surgeon on our face for pete's sake!!!...not to much to ask for, I hope!".

That sounds great. Unfortunately, you will have to find the surgeon that is the right fit for YOU. No one else can do that. You can help and guidance, but YOU must make the final decision.

I will PM you my opinions on several of the surgeons you mentioned in your post.

certification

Thats is really helpful - especially the advice about staying in the US for the surgery...it helps me eliminate one doc off the list! I read somewhere, by another perspective nose job receiver, that once they compiled their list, they would remove any doctor that had any strike or botched surgery on their record...it seems like that would eliminate every dr though!

Anybody have thoughts on whether it matters if a doc has 2 board certifications verse one? and if it is only one is it better to be certified in Otolaryngology verse board certified by the American Board of Facial Plastic and Reconstructive Surgery?

Every doc would not be eliminated, as I know of one on your list personally that has never had any complaints reported (remember that does not mean none exist :)).

You want your plastic surgeon to be board certified by the AAFPRS. Being certified in otolaryngology is a plus, but it should not be your primary criteria. If your rhinoplasty is being done to correct actual medical problems, double board certification would be desirable. If your surgery is elective, AAFPRS certification is what you want, and should require.

Thanks for the additions - I did have David E. Kim - LA on my list, I forget why I took him off actually. Any thoughts on the original list?

I like Frankel's aesthetics, the noses are refined but still natural--I know he's on the pricier side but maybe he's worth it. Pastorek does closed rhinoplasties I believe and I've never seen pictures but I hear he is subtle. Calvert has done some T revisions. I've seen two results by Portuese on MMH and loved them, very natural (was even considering him but I may just stay in CA). Dr. Frank Ryan is a big TV doc and I usually shy away from those because of their inflated prices. I've read a few peoples experiences about Toriumi on rhino boards and I personally would not go to him. It may be true that he has a high level of expertise or has contributed to medical research but after reading more than one story and seeing photos of patients who not only have aesthetic but functional issues (like receiving a "caveman-tube stiff nose", or having a piece of rip stored behind their ears which prevents them from wearing shades or glasses, or having their nose completely and unnecessarily rebuilt with rib against their will, or having their surgery dramatically done in a way that's unfixable) I wouldn't take the risk unless you're needing a really complex reconstructive type rhino.

consultations

So I set up a few consultations, I am hoping I can start getting a better idea of which doc to choose when I meet the surgeons and speak with them. I am still hoping to get some feedback from anyone who has first hand experience with any of these docs...is there anyone out there???????????
My first consultation is tomorrow!...I'm not even sure what I should ask...or should I just listen?

Oh, and how come some docs charge for consultations and others don't? I'm certainly more willing to meet with the free ones easier but does a free consultation mean they are not as good?...I keep hearing my mothers voice saying " No one does anything for free!"...Its getting me sort of worried!
