(1) Many reward credit cards offer promotional reward points for the first 3 months. Often if they normally give 1% cash back they throw in a promo rate of anywhere from 3 to 5% cash back for those 3 months. Should be able to Google some up
(2) Read the fine print. Make sure there's not monthly or annual caps on the rebate and if you have to get on with a cap, get the one with the highest. Often reward cards will limit how much cash back you can earn in a year but others don't. So just make sure you choose one that has no restrictions (or as little as possible) on those promo cash back rate earnings.
(3) Pay for your entire surgery, anesthesia, operating room, etc with the card and bingo - you're paying 95% of the price.
I would like to point out however this should be used with caution only if you can manage your credit. If you plan on carrying this as a balance on not paying it off right away you can forget the gains because reward credit cards always have higher interest rates. And a tiny 5% discount is never an excuse to spend more than you normally would.
